Who we are

This website belongs to Xavier Lima Physiotherapy, trading as X Mobile Physio, ABN 53 469 179 501. Xavier Lima is the owner and the only practitioner.

What the booking form collects

The booking form on this site asks for your name, phone number, suburb, and optionally your email address, how the visit will be paid for, the days or times that suit you, and a short note about what is going on.

Please keep that last box general. You do not need to put any medical detail in it. We will go through your history properly at the first visit.

How that information is used

It is used to contact you, arrange a visit and give you a cost. It is not used for marketing, it is not sold, and it is not shared with anyone else except where the law requires it.

Where the form goes

The form is delivered by Formspree, a form handling service, which passes the message to our email inbox. The website itself is hosted by Cloudflare. Both providers hold the information only to deliver it.

Cookies and measurement

This site counts visits so we can see which pages people actually use. Two tools do that:

Cloudflare Web Analytics. Counts page views and how quickly pages load. It sets no cookies and does not follow you to other sites.

Google Analytics. Sets a cookie in your browser so that several pages viewed in one sitting are counted as one visit rather than several. It records the pages you looked at, roughly where in the world you are, and whether you arrived from a search or a link. Your address is shortened before it is stored, so it does not identify you.

Neither tool is given your name, your phone number, or anything you type into the booking form. We do not run advertising, we do not build profiles, and we do not sell any of this.

If you would rather not be counted, most browsers can block cookies or send a do not track signal, and browser add ons can block both tools. Nothing on this site stops working if you do.

Clinical records

If you go ahead and become a client, the clinical records from your visits are kept separately from this website, under the health privacy rules that apply to registered health practitioners in Queensland. Those records are kept for as long as the law requires and are not accessible from this site.

Seeing or correcting your information

You can ask what we hold about you, ask for it to be corrected, or ask us to delete an enquiry you no longer want on file. Ring 0494 436 802 or email info@xmobilephysio.com.au.

Complaints

If you are not happy with how your information has been handled, please raise it with us first. If that does not resolve it, you can contact the Office of the Australian Information Commissioner at oaic.gov.au.

Changes

If this policy changes, the new version will appear on this page.
